Clinical Documentation Components

A web-based application that integrates with Orion Health Care Pathways supports the curation and management of the authoritative medication list. Embedded in the current date view of medications, within clinical documents and pathways, enabling configuration of a variety of medication centric clinical documents and workflows. These include transfer of care documents, pharmacy care plans, medicines use review documentation and clinical referrals.

Global Drug Model

The Global Drug Model is an application designed to manage SNOMED medication terminologies, drug formulary configurations, pharmacy product and generic concept relationships. Ingested and made available via FHIR APIs, relational terminologies and simple drug files from local/in-region sources support the complex requirements of medication management applications.

FHIR Medication Statement

Stored within the Medicines solution, the FHIR Medication Statement API allows users to retrieve a patient’s current medication list and administration schedule details.

Medicines Viewer

The Medicines Viewer presents a consolidated view of all medications data, including externally sourced messaged prescription orders, dispense fill records, administrations, payer claims data and medication lists from externally sourced documents; combined with the curated authoritative current medication list. Coded medication data normalised to standard terminology concepts as applicable to the region, for example RxNorm, DM+D, AMT, and NZULM. Clinicians can search and filter the list of available data, and group by normalised medication name to reduce the clutter when reviewing a patient’s medication history.

Medication Management

Supports clinicians within the acute care setting by providing reconciliation and review workflows, integration with external medication sources, admission to discharge transfer of care management, prescription generation and discharge medication schedules.

Community Medicines

Enables community-based care providers to actively manage the patient’s medication record and associated Care Plan. Supported workflows include medication reconciliation and review, management of self-prescribed over-the-counter medications and medication adherence progress.

Medicines Viewer

The Medicines Viewer provides a consolidated view across all medication information in the platform. Providing grouped and filtered normalised medication data and prescription records. Information privacy with break the seal are enforceable.

Medicines Decision Support

Amalgamating information from a back-end service and from multiple sources, Medicines Decision Support then displays this information at the point-of-care to help guide in the clinician’s decision-making process. Alerts include duplicate therapy checks, drug monographs for example NZULM, formulary preferences and configurable drug additional information.
